A number of results in the original paper were removed so that the paper could conform to the AJP's paper length constraints.  While this did not affect the overall content of the paper, we feel that for completeness we should provide access to the extended version of the paper here for download.  You can find the entire extended paper here in PDF as well as the two sections that were cut: an extended Section III with momentum-space calculations and the Appendices which supplied additional calculations and problems.

In addition, a number of additional images can be found on this site to complement the images that can be found in the paper.
